School of Education Key & Access Request Form 2018 (PDF)
Use this form to request keys or access to a School of Education space. The form must submitted by a supervisor to the appropriate School of Ed Local Access Controller, who are listed on page two of the form.
Education Building – Grad Lounge Request Form
Use this form to request access to the Graduate Student Lounge at the Education Building. The Grad Lounge is available to any graduate student currently enrolled in a School of Education degree programs. School of Ed grad students may submit these requests themselves.

Use this form to request master level keys to a School of Ed building or building area. The form must be submitted by the department chair/unit director to the School of Ed Local Access Controller. Contact information is on the form. If approved, an employee must pick up the master key(s) in person at the UW Lockshop.
